Auction Item Donation Details for Volunteers
This document provides some background information for volunteers that are inexperienced or uncomfortable about asking for auction item donations from the business community.

Day Of Flight Information Guide – Volunteer Edition
The Day of Flight Information Guide provides volunteers with an understanding about what goes on leading up to the trip to Disneyland with the children, how the groups are structured, what happens in Disneyland and when the children should be back from the day trip. There are two versions of this guide. A Family Edition and a Volunteer Edition. This volunteer edition provides more about the rules of conduct on the trip and a more thorough breakdown on what happens on the day of the trip.

Donation Request Letter
Whenever you approach someone for a potential donation it is helpful to have a copy of the Donation Request Letter with you. Some businesses will ask for a copy of the letter for their record. If someone is considering donating but is uncommitted, you can always write your name and phone number on the letter. You should follow-up with the business if you do not hear back from them. The follow up could be over the phone if you have already left a letter. The donor can potentially receive a tax receipt and we will recognize the individual as well as the donation that was on our website.

DTF Calgary Trifold Brochure
The trifold is a two-sided brochure tells the story of Dreams Take Flight Calgary. If you know people that are asking questions about the charity, but do not know how to answer, print off the trifold and hand it to them. The trifold will also contain contact information for those people that have questions not addressed in the brochure.

The Volunteers Guide is available to help new volunteers better understand how things work throughout the year and where there are some opportunities to volunteer.

Forms
Code of Conduct Form
All volunteers must read and sign the Code of Conduct form. Typically you will have to complete this form every 3 years, but the form is there to remind you of what the rules are. The Code of Conduct rules are set at the National level. All Dreams Take Flight volunteers across Canada are expected to read & sign this form.

Donation Form
A Donation Form must be filled out for auction item donations from a business or individual. This is a fillable pdf document. You can fill out the form online and then print it off or you can print it off and complete the form. Up to 3 different auction items can be identified on a form. If there are more than three unique items being donated and they could be considered being auctioned off separately, then another form should be completed. It is important to fill the form out as completely as possible. Get the name of the person donating the item and the “mailing” address. A Thank You card will be mailed out to the individual and to that mailing address. Try and get the retail value of the items being donated – Do Not Guess! We will be using this information to value the auction items.

Donations As Gifts Form
The Donations as Gifts form is used when someone wants to make a donation “on behalf” of someone else. Occasionally people will ask that friends and family members donate to their favourite charity instead of sending flowers when there has been a death or serious illness in the family. Some people like to make a donation to a charity when there is a special event going on such as: a wedding, graduation or birthday.

Membership Application Form
All members are volunteers, but not all volunteers are members. There is an annual fee of $10 to be a member of the charity. As a member you have voting privileges at the Annual General Meeting.

National Expense Form
Volunteers will be reimbursed for any pre-approved expenses they make on behalf of the charity. An Expense Form must be filled out and submitted to the Treasurer before expenses will be reimbursed. Sales receipts should be attached with the Expense Form or scanned and sent along with the completed form. You can download the Excel version of the form or the pdf version. The Excel version will total the columns for you.

Personal Contribution Form
Individuals or businesses that are making a financial contribution to the charity can fill out this form. It provides us with the information we need to complete tax receipts, if they requested. It also allows the donor to specify how they would like their donation used.
